Application Checklist for Schools
It is the responsibility of the school setting to gather the evidence and submit the application to the LHUB.
Please ensure that you send as much evidence as possible with your LHUB application.
The panel must see evaluated evidence of the schools graduated approach for a child: evidence of support and intervention at a school level, beyond that of a report from a SALT.
The panel must be sure that the school has exhausted their support at a universal/targeted level and the child needs a more specialist, immersive, language environment.
Please remember:
| Complete all parts of the application form in as much detail as possible. | |
| Ensure you have added in all relevant data on the application form. | |
| Ensure the form is signed by the child's parent/carer. | |
| Evaluated evidence of Communication and Interaction support at a school level is included with the application. E.G. play plans, PPPs, IEPs etc to show how the school supports the child on a day to day basis. | |
| An up to date report from a SALT, recomending the LHUB is included with the application. |
