To access a place in the LHUB the referral form below must be completed by the Home School and signed by parent/carer and emailed to Northwood, with the r4levant accompanying evidence to support the application. 

Applications to the LHUB must be led by the school that the child attends. 

Applications then go the the LHUB panel, which meets termly throughout the year. 

Applications must be received by panel no later then 1 week before the date of the panel so that panel has a chance to read and consider them carefully before the panel.
